Alcohol: The Most Well-Documented Gout Trigger

Alcohol is the dietary factor most consistently linked to gout across all epidemiological studies. The relationship is dose-dependent: higher consumption equals higher risk. But not all alcohol is equal — beer, wine, and spirits affect gout through different mechanisms and with different magnitudes.

The Three Mechanisms of Alcohol-Induced Gout

Alcohol raises uric acid through three pathways: (1) Ethanol metabolism increases purine nucleotide turnover in the liver, generating more uric acid. (2) Alcohol's metabolite, lactate, competes with uric acid for kidney excretion, reducing the body's ability to clear uric acid. (3) In beer specifically, brewer's yeast contributes its own purine load — adenine and guanine from the yeast cells used in fermentation add to the purine burden.

Beer: Triple Threat

Beer combines all three mechanisms: alcohol, lactate competition, and yeast purines. The yeast purine content varies by beer type — unfiltered and craft beers tend to have higher yeast content and thus more purines. Studies consistently find the strongest gout association with beer consumption, with some showing a 2.5x risk increase for daily beer drinkers compared to non-drinkers.

Wine: The Weakest Association

Wine shows a weaker gout association than beer in most studies. The Health Professionals Follow-Up Study found that two glasses of wine daily did not significantly increase gout risk, whereas two beers daily nearly doubled it. Possible explanations include wine's polyphenol content (resveratrol has anti-inflammatory properties) and its lower purine content compared to beer. However, heavy wine consumption still raises risk.

Spirits: Purine-Free but Not Risk-Free

Distilled spirits contain no purines — the distillation process removes them. However, the alcohol content (often 40% ABV or higher) means a smaller volume delivers a larger alcohol load. Two ounces of spirits delivers roughly the same alcohol as one beer or one glass of wine, but in a much more concentrated form. The purine-free nature of spirits makes them theoretically lower-risk than beer, but they are not safe.

ACR 2020 Recommendations

The American College of Rheumatology recommends limiting all alcohol intake for gout patients and completely avoiding alcohol during acute flares. The guideline does not distinguish between beer, wine, and spirits — all are considered risk factors. For patients who choose to drink, the evidence suggests wine in moderation carries the lowest relative risk.
