The heat dome parked over the central and southern United States this July pushed heat-index readings past 100°F for more than 40 million people, and the National Weather Service kept extreme-heat alerts running into the final week of the month. With National Watermelon Day landing on August 3, chilled fruit is about to become the default snack in a lot of American kitchens. For anyone managing gout, that raises a fair question: fruit is hydrating and rich in potassium and vitamin C, but it also carries fructose — the one sugar with a documented link to serum uric acid. The honest answer is that fruit is not a single category, and the distinction that matters is not the one most people expect.
Purines and fructose are two different problems
Most gout food advice is built around purines — the nitrogen compounds that break down into uric acid when your body metabolises them. That framework works well for meat, organ meat, and certain seafood. It works poorly for fruit, because fruit is almost uniformly low in purines. A cup of watermelon, a peach, a handful of blueberries: these all sit at the bottom of the purine table, typically well under 20 mg per 100 g.
Fructose raises urate by a separate route. When fructose is phosphorylated in the liver, the reaction consumes ATP rapidly and depletes intracellular phosphate. That triggers degradation of adenine nucleotides, which feeds directly into the purine breakdown pathway and produces uric acid endogenously. In other words, fructose does not contain purines — it prompts your own cells to make more urate. The practical consequence is important, and it is frequently overstated in both directions. The strongest evidence linking fructose to gout involves sugar-sweetened beverages and added-sugar sources, not whole fruit. Choi and Curhan's 2008 BMJ analysis found a clear dose-response relationship for soft drinks and for fruit juice, with a weaker signal for whole fruits such as apples and oranges. Whole fruit arrives with water, fibre, potassium, and polyphenols, and it is eaten slowly. A 20-ounce soda delivers a comparable fructose load in ninety seconds with none of that context.
Where common summer fruits actually sit
Fructose content varies more between fruits than most people assume. Grouping by approximate grams of fructose per typical serving gives a more useful picture than a simple “fruit is fine” or “fruit is risky” verdict.
- Lower-fructose options: strawberries, raspberries, blackberries, cantaloupe, honeydew, apricots, kiwi, and most citrus. Berries in particular deliver a lot of volume and polyphenols per gram of sugar.
- Moderate: watermelon, peaches, nectarines, plums, pineapple, blueberries. Watermelon is roughly 91% water, so a generous 1-cup serving carries only about 5 g of fructose despite tasting intensely sweet.
- Higher-fructose: mangoes, grapes, cherries in large quantities, pears, apples, and above all dried fruit. Drying removes water and concentrates sugar dramatically — a quarter-cup of raisins is not nutritionally equivalent to a quarter-cup of grapes.
Watermelon deserves a note ahead of August 3, because it is routinely miscast. Its glycaemic index is high, but the glycaemic load of a normal serving is low precisely because there is so little carbohydrate in all that water. For someone in a heat wave who needs fluid and potassium, watermelon is a reasonable choice. The risk is not the fruit itself but the format: watermelon juice, watermelon agua fresca with added sugar, or frozen fruit blends that concentrate several servings into one glass. Bananas illustrate the same principle from the other direction. They are frequently recommended for gout because of their potassium content, and they are genuinely low in purines, but a large ripe banana carries meaningfully more sugar than a cup of strawberries. Neither fact makes it a problem food; both are worth knowing if you eat three a day.
What hydration has to do with urate in a heat wave
The heat angle is not incidental. Uric acid is excreted primarily by the kidneys, and dehydration reduces urine volume and can transiently raise serum urate concentration. Clinical guidance for gout consistently includes adequate fluid intake, and periods of heavy sweating — outdoor work, sport, or simply a week of 100°F afternoons — are exactly when intake tends to fall behind losses. Water-dense fruit contributes to total fluid intake, which is one of the few genuinely uncontroversial reasons to eat more of it in summer.
Two cautions belong alongside that. First, dehydration is a plausible flare trigger but not a proven single cause; flares are multifactorial, and the evidence base here is largely observational. Beer and sugary sports drinks both have far stronger associations with gout risk than any whole fruit does, and alcohol adds its own urate-retaining effect on top of fluid loss.
Food safety is part of the summer picture too
One current story is worth folding in. CDC surveillance data released on July 28, 2026, reported 6,707 laboratory-confirmed domestically acquired cyclosporiasis cases across 45 states since May 1, with more than 11,500 additional suspected cases awaiting confirmation. Cyclosporiasis is a parasitic intestinal infection transmitted through contaminated food or water, and fresh produce is a recurring vehicle. The US season typically runs from May 1 through August 31.
This has no direct bearing on uric acid, but it has an indirect one that people with gout should not dismiss. A severe gastrointestinal illness means fluid loss, reduced intake, and sometimes interruption of routine medication — a combination that can destabilise otherwise well-managed gout. Wash produce under running water, scrub rinds on melons before cutting (the knife carries surface contamination into the flesh), refrigerate cut fruit promptly, and follow FDA and CDC recall notices during the summer months.
A workable approach for the next four weeks
Nothing here requires a special diet. It requires choosing formats deliberately.
- Prefer whole fruit over juice. This is the single change with the clearest supporting evidence. Juice removes fibre and makes it trivial to consume four fruits' worth of fructose at once.
- Anchor on berries and melon. High water content, high polyphenol content, modest sugar per serving.
- Treat dried fruit as a concentrated food. Portion it the way you would portion nuts, not the way you would portion grapes.

Should I avoid fruit altogether during a flare?
There is no evidence supporting the elimination of whole fruit during a flare, and doing so removes a useful source of fluid, potassium, and vitamin C at a time when hydration is helpful. Current management of an acute flare centres on prompt anti-inflammatory treatment prescribed by a clinician, not on dietary restriction, and the 2020 American College of Rheumatology guideline explicitly notes that diet alone produces only modest changes in serum urate. What is reasonable during a flare is to limit high-fructose beverages, alcohol, and large portions of purine-dense meat or seafood. If flares are frequent or severe, that is a conversation about urate-lowering therapy rather than about fruit.
